Patient's Query
Hello doctor,
My wife’s rheumatoid arthritis has been an ongoing and challenging struggle for us, and it has been quite confusing. She has been undergoing treatment for the past two years, but unfortunately, nothing seems to be working or helping her improve. Her recent laboratory results are concerning: her ESR is at 65 millimeters per hour, her CRP level remains persistently high at 6.2 milligrams per liter, and her RF is elevated at 135 IU/ml.
Over the years, she has tried several medications, including Methotrexate, Sulfasalazine, and multiple biologic drugs, but her symptoms have not improved. Recently, an MRI showed ongoing joint erosions and increasing inflammation in her wrists, knees, and shoulders, which has further worsened her condition. Every day is a struggle for her as she is in constant pain, her morning stiffness lasts for over three hours, and her joints are swollen and tender. Watching her lose hope has been devastating for me.
Despite the doctors continuously adjusting her medications, nothing seems to provide relief. We are desperate for answers and are wondering if there are alternative approaches to managing her condition or, at the very least, improving her quality of life.
Kindly provide your guidance.
Hello,
Welcome to icliniq.com.
I read your query and understood your concern.
Since she has not responded to basic rheumatoid arthritis (RA) medications, it may be time to explore alternative strategies. Advanced treatments, such as Janus kinase (JAK) inhibitors like Tofacitinib, have shown promise for patients with refractory rheumatoid arthritis. These medications target specific pathways in the immune system and can be effective when other therapies fail.
Beyond medication, I suggest she follow an integrated approach as mentioned below for additional relief:
Physical therapy can help maintain joint mobility and reduce stiffness, while occupational therapy can teach techniques to minimize strain during daily activities.
Pain management specialists can explore options such as localized injections or nerve blocks for severe pain.
Lifestyle modifications also play a critical role. An anti-inflammatory diet, rich in omega-3 fatty acids (found in fish, walnuts, and flaxseeds) and antioxidants (in fruits and vegetables), may help reduce inflammation. Gentle exercises like swimming or yoga can improve joint function without adding stress.
It is vital to keep hope alive. Advancements in rheumatoid arthritis treatment continue, and with the right support, it is possible to improve her comfort and outlook.
I hope this has helped you.
Kindly follow up if you have more doubts.
Thank you.
