How does Tofacitinib differ from biologics or DMARDS?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

After trying Methotrexate without much success, my rheumatologist is recommending Tofacitinib (Xeljanz) as the next treatment. How is this drug different from biologics or other DMARDs (disease-modifying antirheumatic drugs)? I am also concerned about the risks of Tofacitinib, such as infections or blood clots, and how often I will need to be monitored while taking it. Are there any lifestyle changes I should make to reduce these risks while using this medication?

Kindly help.

Hello,

Welcome to icliniq.com.

I understand your concern.

Tofacitinib is a synthetic DMARD that works differently from biologics. It targets Janus kinase (JAK) enzymes, which play a key role in inflammation, rather than targeting specific immune proteins. This helps reduce joint inflammation.

Side effects: Tofacitinib carries some risks, such as

  1. An increased chance of serious infections (for example, pneumonia and shingles) due to its effect on the immune system.

  2. It is also linked to blood clots, especially at higher doses, and may raise cholesterol levels. Regular blood tests to monitor liver function, blood counts, and cholesterol are important.

To minimize risks:

  1. Avoid contact with sick individuals.

  2. Make sure your vaccines, like flu and pneumonia shots, are up to date before starting the medication.

  3. A balanced diet, regular exercise, and quitting smoking can help reduce the risk of blood clots and other side effects.

I hope you are satisfied with my answer. For further queries, you can consult me at icliniq.

Thank you.
