How to manage infection in a root canal-treated tooth?

I have an infection in the tip of my root canal-treated tooth, which was done about 20 years ago. What could be the reason?

Your X-ray would have further been helpful for me in commenting on anything. If you are not satisfied with your present dentist's diagnosis, I would rather suggest you get it physically checked by another dentist before any permanent changes are done.
