Hello doctor,
I have been spitting out foamy and watery saliva after taking my medications, which included IV Omeprazole 40 mg, IV Flagyl 500 mg, and Amoxicillin 500 mg.
I have completed 49 doses in total, and it has been more than two weeks since finishing the medications. I am unsure if this is a side effect, and I would like to know if there is any way to stop it.

Spitting out watery or foamy saliva, long after finishing intravenous (IV) antibiotics and intravenous Omeprazole, is not a usual direct side effect. These drugs are usually cleared from the body within two weeks, so persistent symptoms are likely due to other causes.
It is possible that the medications triggered gastritis or acid reflux, which can cause excessive frothy saliva or a watery mouth sensation.
Strong antibiotics like Metronidazole (flagyl) and Amoxicillin can disturb stomach acid balance and gut flora, sometimes leading to prolonged reflux symptoms.
The probable causes include post-antibiotic gastritis or reflux-related excessive salivation.
Investigations such as upper gastrointestinal (GI) endoscopy, Helicobacter pylori testing (urea breath test or stool antigen), and basic blood work, including complete blood count (CBC) and liver function tests (LFT), can help confirm the diagnosis.
Differential diagnosis includes oral thrush secondary to antibiotics, gastritis from prolonged intravenous (IV) medications, or, rarely, medication-induced esophagitis. The most likely diagnosis is post-medication reflux with hypersalivation.
For treatment, eat small, frequent meals and avoid oily or spicy foods, and do not lie down immediately after eating. Drinking enough water, using probiotics to restore gut flora, and a short course of antacids may help, but consult your local doctor before starting new medications.
If foamy saliva persists beyond another week, or if you experience heartburn, chest discomfort, white patches in the mouth, or unexplained weight loss, see a gastroenterologist. You can also update your doctor with your symptoms after a few days to track progress.
Preventive measures include taking gastro-protective medications alongside antibiotics if prescribed and using probiotics during and after antibiotic therapy. Avoid self-medicating with prolonged intravenous (IV) courses without doctor supervision to prevent complications.
