Hello doctor,
My 21-month old baby had a head injury as he fell and hit his head on the edge of the table and got 2 staples two weeks ago, and also got staples removed within a week, and he was all fine throughout.
Today, he was running and fell on the floor and cried. I took him up, and he got normal after a while, and later, in five minutes, I noticed a bump below that incision scar as he fell on the same side as the scar. But he was perfectly fine, and went to sleep as it was his bedtime, and had milk. He also reacted when I touched that spot, and also rolled and woke up twice to check on me in three hours of sleep since then.

The swelling you noticed below the previous staple site is most likely a localized scalp swelling (hematoma/bruise) caused by the recent impact. It is common for children to develop a bump after hitting their head, and the scalp can swell noticeably even after a minor injury.
The images provided (attachments removed to protect the patient's identity) are unfortunately not clear enough to assess the swelling or the previous wound site.
For now:
Observe him closely for the next 24 to 48 hours.
Apply a cold compress wrapped in a cloth for 10 to 15 minutes at a time during the first 24 hours, if tolerated.
Avoid rough play for the next day or two.
Monitor the size of the swelling and the appearance of the old wound.
Please seek urgent medical evaluation if any of the following occur:
Repeated vomiting.
Excessive drowsiness or difficulty waking him.
Loss of consciousness.
Unsteady walking or abnormal behavior.
Persistent inconsolable crying.
Weakness of any limb.
Rapidly increasing swelling.
Bleeding, discharge, or reopening of the previous wound.
To guide you further, please let me know:
1. Approximately how large is the bump (pea-sized, grape-sized, or larger)?
2. Is the swelling soft or firm?
3. Is it directly over the old scar or adjacent to it?
4. Has he had any vomiting, unusual sleepiness, or behavioral changes since the fall?
At present, based on the history provided, this sounds most consistent with a minor scalp hematoma, and careful observation at home is reasonable if he continues to behave normally.
