Can you please suggest medicine for scalp psoriasis, dandruff, itching, and inflammation on the scalp? I am using only Ketaconazole shampoo. Please suggest oral tablets.

Using just Ketoconazole shampoo in case of psoriasis is not sufficient, as this is an inflammatory condition and needs a combination of topical steroids for at least two weeks, along with a combination of shampoos like coal tar-based and Ketoconazole or Zinc. I understand how uncomfortable and frustrating scalp symptoms like dandruff, itching, and inflammation can be, especially when they persist despite regular shampoo use. Conditions such as scalp psoriasis and severe dandruff (seborrheic dermatitis) often overlap, and it is important to know that this is not due to poor hygiene. It is a chronic inflammatory condition of the scalp, and while it cannot be cured permanently, it can be very well controlled with the right treatment and patience.

Oral tablets are mainly used to relieve symptoms like itching and inflammation, rather than to treat the root cause of scalp psoriasis.

  1. Coal tar-based plus Ketoconazole shampoo is available. Use it two times a week and use normal shampoo on the remaining days. It helps reduce scaling, itching, and inflammation. Apply it to the scalp, leave for three to five minutes, then rinse thoroughly.

  2. Apply Betamethasone lotion (0.05% Betamethasone lotion) every night for two weeks on the affected areas. This reduces redness, scaling, and irritation. Apply a thin layer directly to the patches, gently massage, and wash your hands afterward.

For oral medicine, take tablet Cetirizine 10 milligrams at bedtime for itching and capsule Cholecalciferol 200,000 International Units (IU) once a month only.

Vitamin D plays an important role in skin health and in regulating the immune system, which is particularly relevant in psoriasis, because psoriasis is caused by an overactive immune response that speeds up skin cell turnover. Vitamin D helps slow down this excessive skin cell growth, reduce inflammation, and support the skin barrier, which can make plaques thinner and less scaly over time.

Avoid scratching or vigorous brushing. Use a soft comb. Gently shampooing and moisturizing the scalp can help prevent dryness. Keep stress under control, as stress can exacerbate psoriasis, and try to expose your scalp to sunlight for a few minutes daily to help manage it.
