Hi doctor,
Over the last year, I took a lot of coke. I am embarrassed and ashamed. It is out of character for me and I broke up with my partner, which cost me my job which I loved. Anyway, I got introduced to it and it made me feel good. It becomes a normal thing to do going out. Anyway, I kept getting blocked noses, etc. Always I had to blow hard to clear it.
One day I blew it and there was a hole there. It is getting bigger and bigger and now my little finger will always go through. I cannot tell you the effect this is having on me emotionally. I never ask anyone for help. I am not addicted to any drugs I am not interested in the stuff whatsoever anymore. Sadly I realized too late for my nose. I do not smoke and only drink often. I am worried as the hole is getting bigger. I cannot sleep with it. The hold is coming closer and closer to the entrance to the nostril. I have told my doctor. She said I have to wait and NHS will give me an appointment soon. I am scared and worried. And everything else to go with it. It is my own fault and I am the first to admit it. But I have realized now and I just need some help.

How was your sense of smell? Was it lost? Is the hole in your nose is in the nasal septum? Is the people around you feel the bad smell from you? What was the reason for your breakup and loss of a job?
Have you been examined by a doctor? Can you share a picture of the hole in your nose? Do you have sleep disturbance due to whistling sounds produced while breathing?
A hole in the nose or nasal septum can be due to various reasons. It can be due to trauma, chronic infection, or drugs.

It is a hole in my septum it is from doing cocaine. It is got gradually bigger and bigger as I have been waiting to been seen by a surgeon. The blocked nose was because of doing the cocaine I think. It would run a lot after each time. I had no idea this would have happened to me. I am stupid and silly and hate myself.
My partner broke up with me after seven years she cheated on me with my best friend. I had a good job and loved it but I had to leave because I could not cope with the hurt and pain. She was my first girlfriend and for eight years straight from school. I have lost my smell. I do not smell stuff strong anymore just mild. I have not been seen by a doctor yet. I cannot really get a picture as it is quite difficult but I would say it is close to 2 cm the hole is. Will they be able to repair this? I have even considered going private and paying myself to get it repaired. Is there any advice you can give me? Every day I am worried and I am so sad. You will make my life better if there is a solution to fix my nose. I do not want my nose to collapse and ruin my face.

For your nasal septal perforation (hole), I would advise you to visit an ENT surgeon as they are the specialist to do septal repair (septoplasty). If you have developed deformity in your nose shape then a plastic surgeon will be the proper doctor to repair the hole and correct nose shape. Repair is important as it will improve your symptoms particularly your sleep disturbance and prevent nasal deformity that can develop later.
And if you want to do it paid then go ahead and visit your doctor. After the surgery, you will feel a lot better.
Meanwhile you can use Salinase nasal drops twice a day in your nose and apply vaseline twice a day in your nose with cleaned little finger and see what your ENT doctor advises you.
