Hello doctor,

I am a diabetic since I was 30 years and have heart issues from 15 years. Had bypass done eight years back. Now, lVEF is 25 %. Now, if I walk 50 to 100 meters, I feel breathless and after motion or urine, again feel breathlessness. Is it related to any kidney issues? And also, please suggest can angiography and angioplasty can happen without any risks? Recently, was admitted due to fluid retention in lungs. In 15 days after discharge, I feel same breathlessness and eructations, burps start continue for 15 to 20 minutes. Kindly let me know is this a heart block or kidney issue.

I reviewed your ECG and would explain that it indicates left bundle branch block. Regarding the shortness of breath, it is related to heart failure. Of course, the renal function may be compromised (as it is very common in heart failure). I would like to know your current therapy. The fluid in the lungs is another common feature of heart failure. An optimization of your therapy could help improve your situation. Regarding angiography or angioplasty, it can be performed in this clinical situation safely, if it is indicated by the doctor. But, first, it is necessary to evaluate your kidneys, by performing renal function tests, in order to be sure that there is no risk from IV contrast.
