Hi doctor,

I have just had an MRI (magnetic resonance imaging) but it could take some weeks until I get the results. So I am hoping to find some answers here as well. I have been having pain in my right shoulder for some months now, mainly during activity that puts some strain on the shoulder. During certain exercises, there is also weakness in the shoulder when the range of movement is around 30 to 60 degrees. It is also painful to sleep on the right side. Before it got this bad I would get this pinching type of pain in the anterior part of the shoulder during push exercises which would only last for the duration of the exercise. It stayed like that for some years, and even with lengthy breaks from working out, it would come back just as it was. I went to the gym for the first time in a couple of months and it had worsened. First, I noticed the weakness in combination with pain that felt like it was radiating down towards the shoulder joint, and then the pain in the shoulder started appearing when doing normal tasks. Sometimes, it does feel like the pain is spreading down the lateral parts of the arm, mainly from the deltoid to brachialis and possibly the triceps.

I do understand your concern.

I cannot see any obvious tear but there is some edema in your right supraspinatus muscle near its insertion on the shoulder bone. This could be secondary to some inflammation, especially since you give a history of exercises. The rest of the muscles, tendons, and bones look normal. May I also ask if you have had a fall or acute trauma to the right shoulder?

Hi doctor,

Thank you for the reply.

I have not had any trauma that would cause this, in fact, I had not worked out for a couple of months prior to this except for a few dips once in a while. I have also tried to apply diclofenac over a couple of weeks with no effect. Currently, it is also been 2 months since I last worked out, yet there has not been any improvement at all.

In that case, it looks like tendinitis and I would suggest an MSK (musculoskeletal medicine) or orthopedic doctor’s opinion as they would be able to clinically assess. Do let me know if you have any questions.
