How is facial discoloration with uneven skin tone treated?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My face is discolored, with patches of yellow, red, and brown. I want to achieve an even skin tone without any discoloration. What can be used to improve facial discoloration and obtain an even complexion?

Kindly advise.

Hello

Welcome to icliniq.com.

I understand your concern.

Thank you for sharing the concern in detail. Uneven facial color with yellowish areas on the forehead, brown patches, and dark circles can be distressing, especially when the goal is to achieve a clear and even complexion. Based on the history and the attached image (attachment removed to protect the patient’s identity), the described pattern appears consistent with facial hyperpigmentation with demarcated pigmentary lines, which is commonly seen in conditions such as melasma. The positive aspect is that this condition can improve significantly with a structured and consistent skincare regimen.

Morning skincare routine includes:

Begin by cleansing the face with a gentle, non-irritating cleanser containing mild surfactants and ceramides. This helps remove impurities without disrupting the skin barrier.

  • After cleansing, apply a non-comedogenic emollient moisturizer suitable for sensitive skin to maintain hydration and reduce irritation.

  • The most important morning step is the application of a broad-spectrum sunscreen with sun protection factor (SPF) 50 or higher, containing physical or chemical filters such as Zinc oxide, Titanium dioxide, and butyl methoxydibenzoylmethane.

  • Sunscreen should be applied generously to the entire face and reapplied every two to three hours when outdoors, as ultraviolet exposure is the primary trigger for worsening pigmentation and delayed improvement.

Evening skincare routine includes:

  • Cleanse the face again using the same gentle cleanser. For resistant pigmentation, a triple-combination depigmenting cream containing Hydroquinone, Tretinoin, and Mometasone furoate may be used twice weekly only.

  • Apply a very thin layer strictly to the pigmented areas at night. This is a potent formulation and should not be overused. It can be used safely for up to two months under medical supervision.

  • On alternate nights, apply a milder depigmenting cream containing agents such as Kojic acid, Glycolic acid, and Arbutin, which help inhibit melanin production, maintain results, and reduce the risk of irritation.

This combined approach helps treat pigmentation effectively while protecting the skin barrier. Consistency is essential. Visible improvement generally begins after six to eight weeks, with more noticeable evening of skin tone by around 12 weeks. Scrubbing, bleaching agents, and unverified home remedies should be avoided, as these can worsen discoloration. If redness, burning, or excessive irritation occurs, the frequency of active treatments should be reduced, with greater emphasis placed on moisturization and sunscreen use.

Please continue this regimen patiently. Follow-up is encouraged if there are any concerns or questions. Ongoing guidance can help achieve healthier, more even-toned skin.

I hope you are satisfied with my answer. For further queries, you can consult me at iCliniq.

Thank you.
