Patient's Query
Hello doctor,
I have a problem falling asleep. I have woken up every night, several times, since I was young. I have tried Alprazolam, and it helps me very well. What else can I try?
Kindly suggest.
Hello,
Welcome to icliniq.com.
I understand your concern.
Difficulty falling asleep and waking multiple times at night since childhood suggests chronic insomnia, and while Alprazolam may help you fall asleep, it is not a good long-term solution. Alprazolam is a short-acting benzodiazepine that can cause dependence, tolerance (meaning you need higher doses over time), rebound insomnia, memory issues, and daytime drowsiness.
It treats the symptom temporarily but does not correct the underlying sleep pattern, and over time, it can actually make sleep quality worse.
Chronic insomnia is often linked to the following:
Conditioned hyperarousal anxiety traits.
Irregular sleep habits.
Excessive screen exposure.
Caffeine use.
An underlying sleep disorder.
The most effective long-term treatment is cognitive behavioral therapy for insomnia, which retrains the brain’s sleep rhythm and has better long-term results than sleeping pills.
A few lifestyle changes that can help you include:
Maintaining a fixed wake-up time every day.
Avoiding naps.
Limiting caffeine after early afternoon.
Stopping screens 60 to 90 minutes before bed.
Using the bed only for sleep.
Getting out of bed briefly if unable to sleep after about 20 minutes.
If you are currently taking Alprazolam regularly, do not stop it suddenly, as that can cause withdrawal symptoms.
If you would like, tell me how often and what dose you are taking, and whether you feel anxious during the day as well.
I hope this helps.

Thank you.
