What are treatment options for nr-AxSpA?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am 35 years old, and I was recently diagnosed with non-radiographic spondyloarthritis. My doctor explained that it does not show up on X-rays, but I am still confused about how it was diagnosed. How is this different from other types of arthritis?

I am worried about whether it will get worse over time and if it could eventually lead to visible joint damage. What are my treatment options for managing the pain and stiffness, and is there anything I can do to prevent the condition from progressing? Kindly help.

Thank you.

Hello,

Welcome to icliniq.com.

I read your query and can understand your concern.

The difference between non-radiographic and other types of arthritis lies in joint involvement. For example, in osteoarthritis (bone inflammation), which is more of a degenerative condition, X-rays would show the wearing down of cartilage.

In nr-axSpA (non-radiographic axial spondyloarthritis) (active inflammation of the spine and sacroiliac joints), the inflammation is present but without visible joint damage early on. Over time, though, it can progress to ankylosing spondylitis, which can show up on X-rays as the inflammation causes new bone formation, leading to fusion of the vertebrae. You are right to be concerned about whether it could worsen over time. While not everyone with nr-axSpA will progress to ankylosing spondylitis, the risk is there, and that is why early treatment is crucial.

Treatment is to manage symptoms like

  1. Pain.

  2. Stiffness.

  3. Reduce inflammation.

  4. Prevent joint damage.

In addition to medication, I suggest the following

  1. Regular exercise, particularly stretching and strengthening exercises, can help keep your spine flexible and improve posture.

  2. Physiotherapy can be extremely beneficial in managing the condition.

  3. Diet can also play a role in reducing inflammation, so focusing on anti-inflammatory foods could be helpful.

Regular follow-ups with your rheumatologist can help.

I hope this information will help you.

Thank you.
