What causes irregular periods and bleeding after sex?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I am a 28-year-old female and have normal periods every four weeks. However, my period was four days late last month and lasted only three days, and the last day was spotting and very light.

The last two times I have had sex, I have bled, and when masturbated with no preparation, I bled. It is not bright red, but was browner. And does not require me to wear anything for the most part. It also does not last long. I took a pregnancy test today and got negative. I am not on birth control. Any idea why I am bleeding? If my period goes back on track from my last one, I should be in the middle of my cycle.

The last answer I got was, if the test is negative, then wait for the pds. It will come. But I do not know what pds stands for.

Kindly advise.

Hi,

Welcome to icliniq.com.

It seems there has been a recent disturbance in your cycle. Irregular, unscheduled spotting could be due to an underlying cause like cervical or endometrial polyp, cervical erosion, hormonal imbalance, any illness, drug intake, etc.

Have serum beta hCG quantitative human chorionic gonadotropin (blood pregnancy test) immediately. If it comes positive, immediately have a transvaginal scan to see if a retained product needs evacuation. If it comes negative, then follow the treatment advised below.

1) Have cervical smear, patient local hysteroscopic examination by a gynecologist, high vaginal swab and transvaginal scan, thyroid function test.

2) Have a healthy diet. A daily walk, balanced BMI (body mass index), and weight.

3) Collect laboratory results and discuss them here.

4) If irregularities remain, then you can have sex hormonal tests (serum progesterone, testosterone, insulin, prolactin, FSH (follicular stimulating hormone), and LH (luteinizing hormone) On day two of periods as well. Check for abnormal hair growth and sudden weight gain and weight loss well.

5) Have a healthy diet, daily walk, exercise, and balanced BMI (body mass index) and weight.

6) If menstrual irregularities continue, you can have combined oral contraceptive pills for three months cyclically.

I hope this answers your query.

Thank you.
