Why am I having painful urination and spotting after sex?
Patient's Query
Hello doctor,
I spotted a small amount of blood in my vagina this morning, so I thought I was menstruating, but it was only a spot. I am really experiencing painful urination right now. I had an abortion one month ago, and I had my second sexual intercourse two days ago.
Kindly help.
Hello,
Welcome to icliniq.com.
I understand your concern.
The most likely diagnosis is acute urinary tract infection (UTI) or cystitis, possibly triggered after sex. The painful urination that started soon after sex strongly suggests a urinary tract infection (UTI) or urethral irritation from intercourse. This is very common, especially when symptoms begin within one to three days after sex. The small spot of blood could be from:
-
Mild vaginal spotting (your period may be starting).
-
Irritation from sex.
-
Sometimes, blood in urine is from a urinary tract infection (UTI).
Because you had an abortion one month ago, it is also important to rule out infection related to the procedure or a new pregnancy, especially if the sex two days ago was unprotected.
Other possibilities to consider include urethral or vaginal irritation from friction during sex, sexually transmitted infection (STI), such as chlamydia or gonorrhea, if sex was unprotected, and, less commonly, post-abortion infection if you also have pelvic pain, fever, or bad-smelling discharge. Please get a urine test (urinalysis and urine culture) as soon as possible. A pregnancy test is also a good idea if the recent sex was unprotected.
Important note about Metronidazole: it is not the usual treatment for a simple urinary tract infection (UTI). It treats bacterial vaginosis and some vaginal infections, but most urinary tract infections (UTIs) need a different antibiotic.
Seek urgent care today if you have:
-
Lower abdominal or pelvic pain.
-
Foul-smelling discharge.
-
Worsening bleeding.
-
Back pain.
-
Vomiting.
These could suggest a more serious infection after the abortion.
