Hello doctor,

I have right testicular pain (bearable) when pressure is applied at one point. The pain comes and goes, and the color of my semen has changed from white to yellowish.

Intermittent right testicular pain that is mild, localized to one spot on pressure, and gradually improving over time is most commonly related to inflammation of the epididymis or testis, known as epididymitis or epididymo orchitis, especially given your history of a sexually transmitted infection.

A previous syphilis infection and treatment with antibiotics such as Cefixime and Doxycycline suggest that an infectious cause was suspected, and partial improvement supports this. Yellowish semen can occur due to infection or inflammation of the prostate, seminal vesicles, or epididymis, or sometimes due to concentrated semen, dehydration, or residual infection.

Niftas and Alkasol mainly act on the urinary tract and may not fully treat genital tract infections, which could explain persistent mild symptoms. Although the pain is now reduced, ongoing tenderness means the inflammation may not have fully resolved, or there may be another cause, such as chronic epididymitis, prostatitis, or, less commonly, only a cyst or varicocele.

It is important not to ignore testicular pain, even if bearable, because conditions like torsion are emergencies, though your presentation does not strongly suggest torsion since the pain is chronic and improving.

You should consult a urologist for a proper genital examination, and you may need a scrotal ultrasound with Doppler, Doppler ultrasonography, culture, semen analysis, and repeat STI (sexually transmitted infections) testing if advised.

Avoid sexual activity until evaluated, use scrotal support, stay well hydrated, and do not self-start antibiotics without medical advice. Prompt specialist evaluation will help prevent chronic pain or fertility-related issues.
