Should I adjust my thyroid medication for elevated TSH levels?
Patient's Query
Hello doctor,
I have been on thyroid medication for around 15 years. Since two months ago I have been taking 67.5 micrograms of Thyronorm. In a recent thyroid test, my TSH level registered at 13.2, while my T3 and T4 levels were within the normal range. In the previous test from October, my T3 and T4 were normal, but the TSH was 14.2. I haven't experienced any symptoms related to this TSH level. Should I consider increasing my medication dosage, or would it be advisable to retest the TSH? Kindly help.
Thank you.
Hello,
Welcome to icliniq.com.
I can understand your concern.
You mention that you have been on Thyronorm 67.5 micrograms for the last 15 years and that your TSH levels have consistently been above the normal range in the two blood tests conducted within the past six months.
If you have not altered your Thyronorm dosage, noticeable changes in your TSH (thyroid stimulating hormone) levels would not be expected. Since the TSH remains elevated, my recommendation would be to increase the Thyronorm dosage to 100 micrograms, take it for 4 weeks, and then retest your TSH. If the TSH returns to the normal range, continue with Thyronorm 100 micrograms daily and monitor your thyroid function through annual blood tests. Should the TSH still exceed 10, then consider increasing the dosage to 125 micrograms and conduct another blood test in four weeks. If the TSH measures below 1.5, it indicates potential overcorrection. In this case, reduce the Thyronorm to 75 micrograms and retest after four weeks.
I hope this information will help you.
Thank you.
