Hello doctor,
I have been experiencing pain on the right side of my head, neck, ear, face, and teeth for the past two years. At times, it escalates into a migraine, and no medication provides relief. I have OPG images with me. Could you help determine whether the cause is a dental infection, a TMJ issue, or pain due to an elongated styloid process? Please help.

A review of her dental history (attachments removed to protect the patient's identity) indicates that she has undergone dental procedures, including an implant. Examination of the OPG (orthopantomogram) images (which is a panoramic dental X-ray that provides a broad view of the upper and lower jaws, teeth, and surrounding structures) shows that the root of the lower right third molar appears underdeveloped and is not in close proximity to the mandibular canal. Additionally, the upper molar, which opposes the lower molar, exhibits pulp chamber visibility.
There are concerns regarding potential TMJ (temporomandibular joint) involvement on the right side, as observed in the OPG scan. Possible issues include reduced synovial space (the gap within the TMJ containing fluid that facilitates smooth joint movement), midline shift (a misalignment of the jaw or teeth, causing deviation from the facial midline), or ankylosis (the fusion of the TMJ bones, leading to restricted or no jaw movement). Further evaluation is needed to determine whether these factors are contributing to her persistent facial pain.
The probable cause is reduced synovial space of the right TM Joint. The differential diagnosis is ankylosis of the right TMJ, midline shift, and adjusting the bite. The probable diagnosis is right TM Joint reduced synovial space and functional loss due to edentulous areas on the right side, which cause bite problems and lead to dysfunction. For treatment, I suggest rehabilitating edentulous areas of the mouth with prosthetic missing teeth replacement to improve function from mastication and chewing, such as speech, improving oral functions, and improving impaired TM Joint function and health rather than causing pain from friction from reduced synovial space. The preventive measures include rehabilitation with the replacement of missing teeth. Have a clinical follow-up, preferably prosthodontist and oral surgery opinion.
