I had an ankle fracture. How to manage twisted ankle pain?

Patient's Query

Hello doctor,

Almost two years ago, I sustained a trimalleolar fracture to my right ankle. Two weeks ago, I twisted the same ankle, and now I have pain in my right lateral malleolus when I walk. Most of the time, I only have pain while putting weight on my foot, but occasionally I have burning pain.

I do not have pain when I put weight on my heel. Is it typical to have pain where the plate is?

Should I be concerned?

I am taking Phenytoin, Fluoxetine, Buproprion, and Insulin medications.

Please help.

Actually, it is difficult to answer your query with the amount of information provided. I need the following information to understand your problem better:

  1. How was the injury managed at that time?
  2. Did you undergo any surgery?

Kindly send your X-ray of the injury you had two years ago, and also a picture of your fractured ankle, so that I can give you the right suggestions.

Hello doctor,

Thank you for your reply.

I did have an ORIF, two screws with a lag screw medially, and a plate laterally. My ankle has no obvious deformity currently, but it swells after I walk around.

For the previous injury, I went to the ER and had my ankle set (took three tries), then had surgery three weeks later.

After surgery, I was non-weight-bearing for three months.

Please help.

I can understand your concern.

It is very important information you have not mentioned previously. So, this time again you have an injury in the same ankle. It is easier to get a fracture in the same bone than in another bone, as already there is a fracture line, though united, there is weakness there.

I suggest you get a new X-ray done. I want to know-

  1. Are you able to bear weight on that limb?
  2. Has swelling reduced with time, or is it increasing?

Hello doctor,

I can bear weight, but it hurts. The swelling has not changed. My ankle feels warm, and I put ice packs on it, but I cannot tolerate the cold on the plate. I will get X-rays in the morning. I honestly was worried that I was overreacting.
