How to manage my uncontrolled diabetes despite medication?

Hello doctor,

I am 54 years old and have had type 2 diabetes for nearly nine years, and despite being on multiple oral medications and more recently insulin injections, my blood sugar levels remain completely uncontrolled.

My latest HbA1c came back at a dangerously high level that my endocrinologist attributed entirely to poor dietary compliance without considering any other potentially contributing factors.

Can a general physician help a 54-year-old type 2 diabetes patient whose blood sugar remains persistently uncontrolled despite maximum medication?

To investigate whether underlying insulin resistance, stress hormones, sleep deprivation, or undiagnosed pancreatic insufficiency could be significantly contributing to her treatment-resistant glycemic control that dietary changes alone will never adequately address.
