Can typhoid return after treatment in a 2.8-year-old?

Hello doctor,

My child is 2.8 years old. Ten days before, she was suffering from typhoid again. Today she has a fever. I am so afraid.

As a child specialist, I understand why you are worried. Since your daughter recently recovered from typhoid fever and has developed a fever again after about ten days, her pediatrician should re-evaluate her.

Although the fever might be a new viral infection (which is very common in children), it is also important to rule out persistent or relapsed typhoid, an incomplete response to treatment, or another bacterial infection.

Please continue to:

  1. Encourage plenty of fluids (breast milk, water, or oral rehydration solution, if needed) to prevent dehydration.

  2. Give paracetamol (acetaminophen) 15 mg/kg (milligrams per kilogram) per dose every four to six hours as needed for fever, with a maximum of four doses in 24 hours.

  3. Monitor her activity level, urine output, and feeding.

I would also recommend assessing her growth and nutrition during the visit because her weight of 8.6 kg (kilograms) is quite low for a 2.8-year-old.

Your pediatrician may recommend investigations depending on her examination and symptoms, such as the following:

  1. Complete blood count (CBC).

  2. C-reactive protein (CRP).

  3. Blood culture should be performed, especially if the fever is high or persistent, and it should be done before starting antibiotics.

  4. Urine examination and urine culture if a urinary tract infection is suspected.

  5. Other tests based on her symptoms.

Please do not start antibiotics on your own, as they can make it harder to identify the cause and may not be the right treatment.

Seek urgent medical attention immediately if she exhibits any of the following symptoms:

Is difficult to wake, very sleepy, or unusually irritable.

  1. Has difficulty breathing.

  2. The patient refuses all fluids and has repeated vomiting.

  3. Has a seizure.

  4. Develops a rash with purple spots or severe abdominal pain.

  5. The patient passes very little urine or shows signs of dehydration.

Kindly answer the following questions:

  1. What is her current temperature, and how was it measured?

  2. Which antibiotic was used for typhoid, at what dose, and for how many days?

  3. Was typhoid confirmed by a blood culture, TyphiDot, or Widal test?

  4. Does she have a cough, runny nose, vomiting, diarrhea, painful urination, or abdominal pain with this new fever?

  5. Is she eating and drinking normally today?
