Can stress worsen my sister’s ulcerative colitis at 32?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My sister is 32 and has suffered from ulcerative colitis for five years. She experiences two to three flare-ups every year despite her regular intake of mesalamine. My sister’s current lab results show she has anemia (Haemoglobin 11 g/dL) and high stool calprotectin. In addition, she suffers from anxiety and a sleep disorder. Kindly advise,

  1. Does stress exacerbate ulcerative colitis?
  2. Is it time to switch her to other stronger medications, such as immunosuppressants?
  3. What are the lifestyle modifications that may help decrease the number of flare-ups?

Kindly help.

Hello,

Welcome to icliniq.com.

I read your query and can understand your concern.

Your sister has ulcerative colitis, with two to three attacks, which implies that she is possibly not under control. The elevated level of stool calprotectin shows that there is an inflammatory process in the intestines despite the absence of obvious symptoms.

Stress and anxiety are not the reasons for having ulcerative colitis, although they may aggravate the existing symptoms. Managing anxiety and improving sleep can therefore help overall disease stability.

Because she continues to have frequent flares, her gastroenterologist may include immunomodulators (such as Azathioprine) or biologic therapies, depending on disease severity and response to current medication. Such interventions seek to achieve deeper remissions and lower frequencies of flare-ups.

Her mild anemia (hemoglobin levels 11 g/dL) could be due to her chronic inflammation or loss of blood; tests for iron levels will help identify if any treatment is required.

Some interventions she could adopt include,

  1. Taking her medications regularly.
  2. Management of stress by exercising or meditating.
  3. Getting sufficient sleep.
  4. Abstaining from smoking and heavy consumption of alcoholic drinks.
  5. Following a good, balanced diet.
  6. Detecting any foods that trigger her attacks.

I hope this information helps you.

Feel free to ask further questions.

Thank you.
