Hi doctor,

I have a cyst on the skin of the upper lips of my vaginal close to where the opening is. It has been there for a while. I thought it was a pimple. But it has not gone away. It is painless, and you can tell something is amiss from the outside. When I touch it, it feels like the size of a pea. It is tiny and almost feels like there is pus under the skin from a pimple. But I can not seem to pop it; it has been there for at least a month. I found it accidentally since it is not painful at all.

I understand that you are concerned about a cyst in the perineal area. From the description, it seems like a Bartholin's cyst which occurs commonly in women who are sexually active and do not need treatment unless infected or very large. To confirm, is it near the labia minora (the inner tags near the vaginal opening)? Also, is it more downwards, towards the anal opening, and not near the urinary opening?

Thank you for the reply.

It is down to the right of the vaginal opening, but it is on the outer skin part of my vagina where there is hair, and it is dry. So it is on the outer part of my vagina where the skin is dry, and there is hair. I have just uploaded a diagram to show you. It is where the purpose dot is. It could be a little lower but on the skin part, not inside.

I understand your concern.

The location you described and in the image (attachment removed to protect the patient's identity) is typical of a Bartholin's cyst. It does not need any treatment and can be left alone. It is basically due to blockage of a gland's duct that secretes vaginal secretions. In case it gets infected, or the size is bothersome, only then would it need to be excised. For symptomatic relief, you could try sitz baths.

So the punctum of the cyst is on the inside, but the bulk of the swelling can be felt from the outside. To be entirely sure, you would need an internal examination to confirm it.
