Hi doctor,

I am a 68-year-old woman experiencing significant knee and back pain. I lead a sedentary lifestyle. Since going through menopause, I have noticed that my joint pain has worsened. I have read that vitamin D deficiency can contribute to these issues, especially in older adults. Given my post-menopausal status and my current symptoms, could my pain be linked to a lack of vitamin D? What kind of tests should I get to confirm this, and what treatment options would you recommend to manage the deficiency and alleviate my pain?

With your age and postmenopausal status, you should get screened for osteoporosis as well as vitamin D deficiency. Vitamin D deficiency commonly affects those with joint pain, but osteoporosis care is especially important for females. Reproductive factors, especially early menopause and estrogen replacement therapy, put postmenopausal women at high risk, as do hysterectomy and oophorectomy. Symptoms of vitamin D deficiency in adults include periosteal bone pain, best detected with firm pressure on the sternum or tibia.

Quantitative imaging methods commonly used are dual-energy X-ray absorptiometry (DXA) and quantitative computed tomography (QCT) scanning. Additionally, get CBC (completed blood count), TFT (thyroid function test), serum vitamin D, and serum calcium tests done. Screening for vitamin D deficiency is recommended only in individuals at high risk, including

  1. Patients with osteoporosis.
  2. Patients with malabsorption syndrome.
  3. Black and Hispanic individuals.
  4. Obese persons (body mass index greater than 30 kilograms per square meter).
