What causes white patches on the penis and pubic area?

Hi doctor,

I have always had small white dot-like patches at the very edge of my penis, specifically in the area between the lower abdomen and the penis. Images are attached. Recently, after shaving my pubic area, I noticed the same white spots there as well. When I apply moisturizer, they fade or disappear, but they become visible again when the skin is dry.

Is this something serious? I had a protected sexual encounter with a CSW a few years back. The condom was intact, and I tied it off after. Doctors at the time said there was no risk from that encounter. I am otherwise healthy and have even gained weight recently.

Please guide.

The condition appears to be a papulosquamous skin disorder, primarily triggered by dryness. It tends to worsen with itching, dryness, and seasonal changes. This is not related to any sexually transmitted disease (STD), and there is no need for concern in that regard. However, as a precaution, a blood test for VDRL (venereal disease research laboratory) is advised. No additional blood tests or skin tests are required at this point. The spots are likely allergic in nature and not due to any fungal or infectious cause.

For treatment, start applying:

1. Eumosone-M cream (Mometasone and Fusidic acid) to the affected area twice daily.

2. 15 minutes after applying the cream, apply Cutimax-O lotion (a moisturizing lotion containing glycerin and light liquid paraffin) to lock in hydration.

This routine should be followed consistently for three weeks. The condition should begin to improve within that time, but treatment will not be stopped abruptly. The medication will be gradually tapered to prevent recurrence.

To help manage and prevent flare-ups, use a mild soap during bathing, and always apply moisturizer immediately afterwards. Avoid scratching or exposing the area to irritants. Once the condition resolves, continue using a moisturizer regularly to maintain skin hydration and prevent recurrence. Please share the VDRL report once available, and plan a review after three weeks to assess progress.

Hi doctor,

I live in a small town where diagnostic facilities are limited. I plan to visit the city in a few days to get the necessary tests done and will update you afterwards. Please note, I have always had these white spots on the same area of my penis, but never noticed them on the pubic region until recently.

Regarding the past sexual encounter: I had both oral and vaginal sex, but it was fully protected with an intact condom. Afterwards, I consulted one of the top dermatologists and venereologists. He assured me at the time that there was no risk. That said, I would like to ask again, do these findings suggest a sexually transmitted disease (STD)?

Your condition does not appear to be a sexually transmitted disease. It seems more likely to be eczematous in nature. The only reason I suggested getting the test done is to rule out any possibility of an STD solely due to that past sexual contact.
