Hello doctor,

I had a complicated surgical extraction of my third molar on the left mandible, along with osteotomy, to better facilitate the tooth removal. Since then, the pain from the tooth has subsided, but now I have trismus and mucosal swelling, and tonight while at work, a copious amount of thick yellow purulent exudate was expelled into my mouth. I did nothing to facilitate this drainage other than using warm compresses over the last several days in hope that it would help with the discomfort caused by swelling. After the fluid drained, there was almost instant relief from the mucosal swelling just lateral to the incision sites of surgery. I am currently taking Augmentin 875 mg twice daily for 14 days, but I fear whether I need something stronger, such as Clindamycin or Vancomycin. I need to know if this is an ordinary occurrence (the pus draining) or if this is a symptom of my infection progressing or some other post-extraction complication. Also, please note that my glands are swollen under the left mandible and the soft tissue under my tongue. Pain is slightly less now after the pus is drained, but now I am so scared that the clot has been lost and a dry socket will form.

If you do not feel pain now, there is no need to worry. Continue using the capsule Augmentin (Amoxicillin and Clavulanate) 875 mg as suggested by your doctor. Use warm salt water gargle twice daily. You need proper exercise to reduce jaw stiffness and increase mouth opening. You can start exercising now, as it has been six days. If there is severe sharp pain, then it means a dry socket condition. Then you need to consult your dentist again.
