I have a red patch on my penis head that will not go away and gets even more red after sex.

It is a form of balanitis called zoon's balanitis or plasma cell balanitis. It is an inflammation of unclear cause but may be due to some allergy or irritant. It causes soreness or burning. You should have mentioned symptoms.

The Probable causes

The probable cause is unknown but irritants aggravate.

Investigations to be done

Do biopsy for histopathology to rule out other differentials.

Differential diagnosis

The differential diagnosis are candidial balanitis, erythroplasia of queret.

Probable diagnosis

The probable diagnosis is Zoon or plasma cell balanitis.

Treatment plan

Use Tacrolimus 0.03 % apply twice daily for three weeks. Use Emollients like Alovera daily. Avoid harsh soaps.

Preventive measures

Clean regularly and avoid irritants.

Regarding follow up

Have a follow-up after three weeks.
