I have swollen veins on my penis, which are very large. I have discomfort, especially after masturbation. They had disappeared for some time, but have recurred. I am 22 years old and have varicose veins on my legs. What can be the problem?

Are high PSA levels and occasional frequent urination suggestive of kidney infection?
Hello doctor,
I am a 31-year-old male with a normal BMI, and no other known health issues, non-smoker, non-drinker, and no drug use ever. My appendix got removed 10 years ago. I have been experiencing high PSA and occasional frequent urination for one year now. The latest total PSA is 9.95. Urine or semen culture is always negative for bacteria so I decided to wait for EPS before taking Ciprofloxacin. I have taken fosfomycin empirically for two weeks last month, symptoms subsided but came back a few days after Abx termination. Lately, feeling a bit under the weather, I do not have but definitely feeling weak and malaise-like. Some flank right-side discomfort is also present. So I decided to do some tests. My concerns and questions are:
1. Is this kidney infection from the lower urinary tract?
2. Why were hyaline cast and renal tubular epithelial cells found?
Seems high amounts, especially RTECS. Very concerned since RTECS with a hyalin cast can suggest something.
3. What do you suggest I should do next? Please suggest further tests and medication. I also read if left untreated, long-term kidney scarring can happen. Hoping it is just an infection that Abx can handle and not something else like acute tubular necrosis. Pleae help.

What is the reason for the increase in my PSA levels?
Hello doctor,
My PSA levels went up from 1.4 to 2.38 during my physical, and the digital rectal examination (DRE) did not find any issues. I have another PSA test scheduled for next month, and I am very concerned. Should I consider requesting an MRI or a biopsy before that test? It is important to note that I had BPH years ago, possibly aggravated by cycling with an uncomfortable seat. I continue to ride regularly, and I can not recall if I rode close to the exam date. My current medications include Lorazepam for stress and Amitriptyline in a subclinical dosage for IMS. Please help.

What should be the line of treatment for a ureteric stone?
Hello doctor,
I am consulting for my wife. My wife was consulting a general physician for a routine check-up. As per her doctor, the urine analysis showed abnormal ratios. As per reports, there is blood in the urine. The second albumin-to-creatinine ratio dropped from the previous level of 22 to 7.50 mg/g, however, it is still high. Later the ultrasound revealed the presence of a right ureteric stone. I have consulted a kidney specialist and he told us to consult a urologist. As per the kidney specialist, there is hydronephrosis on the ultrasound report and it is causing back pressure on the kidneys. Kindly advise on the treatment course.
