Hello doctor,
I am feeling discomfort in my stomach. There is no vomiting or diarrhea, only a lot of burping and abdominal cramping. What should I do to manage this?

You are describing upper abdominal discomfort with excessive burping and a crampy sensation, without vomiting, diarrhea, fever, or other alarm symptoms. In the absence of weight loss, blood in stools, persistent vomiting, or nocturnal symptoms, this presentation is most consistent with functional dyspepsia or gastric indigestion with gas-related bloating. This condition is often triggered by dietary factors, stress, irregular meals, aerophagia (swallowing air), or transient gastric dysmotility.
Excessive burping commonly results from gas accumulation in the stomach rather than excess acid alone. Cramping can occur due to gas distension or increased gut sensitivity, even when routine investigations are normal. Stress, anxiety, hurried eating, carbonated beverages, caffeine, and fatty or spicy foods frequently aggravate these symptoms.
I would like to suggest the following:
Begin with a proton pump inhibitor (PPI) once daily before breakfast, such as Pantoprazole or Esomeprazole, for two to four weeks.
Add a prokinetic anti-gas agent, such as Itopride or Levosulpiride, combined with Simethicone, to reduce bloating and improve gastric emptying.
Avoid carbonated drinks, chewing gum, smoking, and eating too quickly.
Take small, frequent meals and avoid lying down immediately after eating.
Warm fluids and gentle walking after meals may help relieve gas.
If symptoms persist beyond two to three weeks or worsen, further evaluation, such as basic laboratory tests, helicobacter pylori (H. pylori) testing, or endoscopy, may be considered.
Most cases improve with simple medical treatment and lifestyle modification. This condition is benign and reversible, and reassurance plays an important role in symptom relief.
