Why do I still have acne despite using AHA, BHA, and SA?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I currently use Skin Aqua sunscreen, Revox Zitcare AHA, BHA, PHA toner, SA Salicylic face wash, ceramide face wash, and Seoul 1988 moisturizer.

Why do I have bumps on my face, acne, and dark spots?

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I have read your query.

I can completely understand your concern, and I will try my best to guide you to the appropriate management so that you can achieve smooth skin.

Despite using multiple skincare products, your acne, bumps, and dark spots are persisting mainly due to the overuse of exfoliating agents and possible skin barrier disruption. You are currently using a combination of an AHA (alpha hydroxy acid)/BHA (beta hydroxy acid)/PHA (polyhydroxy acid) toner along with a salicylic acid face wash, which can lead to excessive exfoliation, causing irritation, increased oil production, and worsening of acne and post-inflammatory hyperpigmentation (a common, acquired skin condition characterized by dark, flat spots or patches).

At this stage, the best approach is to simplify your routine and allow your skin to recover. You should switch to a gentle cleanser (preferably your ceramide-based face wash), continue using your moisturizer and sunscreen regularly, and temporarily stop the exfoliating toner.

Instead, introduce a topical retinoid such as Adapalene 0.1 gel at night (starting two to three times per week and gradually increasing), as it would be more effective, as it helps in unclogging pores, reducing acne, and improving skin texture and pigmentation over time. After the gel, use moisturizer to avoid dryness.

With consistent use and a simplified routine, you would notice improvement over the next six to eight weeks.

I hope this answers your query.

Please let me know if I can assist you further.

Thank you.
