I am a 17-year-old male who has noticed slight hairline recession at the temples. I have researched treatments such as Finasteride but am still debating whether to use it. Is it necessary to check my natural levels of DHT before using the drug, or should I try it out and see what happens?

I have gone through your history. It may be MPHL or male pattern hair loss. Male pattern hair loss occurs due to multiple factors, including genetic factors. Genetic susceptibility is passed on from the maternal side, including maternal uncles, siblings, or ancestors. Other factors include -

  1. Diet.
  2. Hormonal fluctuations include androgenetic alopecia, where the male androgen hormone is imbalanced.
  3. Fungal infections while wearing helmets and caps.
  4. Spread from an infected individual while sharing belongings.
  5. Dandruff or seborrheic dermatitis.
  6. Vitamin deficiencies.
  7. Hair procedures or treatments, including smoothening, straightening, creams, gels, or hair colors.
  8. Stress.
  9. Androgen level fluctuations are one of the main reasons that may lead to MPHL or also androgenetic alopecia; therefore, tests of the same are always advised. Finasteride can be applied topically; however, you can use oral Finasteride tablets for your condition.

    Consult your specialist doctor, talk with them, and take medications with their consent.

    My advice :

    1. Blood tests: CBC (complete blood count), Hb (hemoglobin levels), serum ferritin, serum iron, serum zinc, serum vitamin D3, serum vitamin B12, DHEAS (dehydroepiandrostrerone sulfate), TFT (thyroid function test), and ANA (antinuclear antibody test). These tests help diagnose any vitamin deficiencies and hormonal imbalances, which may require additional treatment as supplements if present.

    2. Take a multivitamin nutritional supplement once daily after two months. They are multivitamin tablets meant for hair growth and reducing hair fall. They should only be started after providing a blood sample for testing since starting the medication and then providing the blood samples may lead to false results.

    3. Apply 1 ml of Minoxidil Finasteride 5 % solution on the area of hair loss at night on a clean, dry scalp. Apply 1 ml quantity using the dropper available with the medication or 3 to 4 sprays, massage into the scalp, and keep it overnight. Wash off in the morning. Minoxidil with Finasteride helps increase blood supply and circulation to the hair follicles, stimulating their growth. However, they need to be used for years to show changes. Initially, it should be applied once daily for one year, then reduced to six times per week for the next year, then five times per week the following year, and so on. They should not be abruptly stopped as they may lead to sudden cut off of the blood supply to the hair follicles leading to their loss. Therefore slowly tapering down the solution each year is the way to let your scalp get accustomed to the solution. Avoid usage in case of migraine, seizures, major medical conditions, history of allergies, etc. Oral Finasteride is not required. The solution is more helpful.

    Minoxidil solution and Finasteride should be used for at least one year to show results and should be continued as mentioned above.

    4. Procedures including platelet-rich plasma (PRP), growth factor concentrate (GFC), and mesotherapy once a month may help stimulate hair growth. PRP and GFC are injectables done on the scalp superficially using one's blood revolved in a centrifugal machine to extract the growth factors helping to stimulate hair growth. Mesotherapy is injections of vitamins done similarly to stimulate hair growth. Kindly consult a dermatologist in person to undergo the procedure.

    5. Avoid oily food, sweets, dairy products, and red meat. Make sure to include protein, like an egg, in your diet.

    6. Ensure hydration with at least 13 glasses of water (250 ml glass size) daily.

    7. In case of dandruff, apply Ketoconazole and Zinc Pyrithione lotion, one teaspoon diluted in one cup of water and applied once a week on the scalp, massage, and wash off after three minutes.

    8. In case of severe hair loss and full patterned MPHL, hair transplant procedures may be done in the future, which helps in transplanting hair follicles in areas of loss of hair follicles.
