Hi doctor,

So when I pleasure myself and when I cum, I feel a tingling sensation. I think like I cannot go on, but when I have sex, I do not know if I cum or not. I feel pleasure, but I do not know what to expect, and I do not know whether I cum or not? It has been present for a year (from when I started having sex).

I can understand your concern.

Your symptoms are suggestive of anejaculation. Anejaculation can be due to,

Retrograde ejaculation – semen will not come out; it will return to where it comes from.

• Increased blood sugar. So check for blood sugar levels.

• Prostate issue. So plan for an ultrasound pelvis.

• Renal stone. So plan for an ultrasound abdomen.

• History of surgery.

• On any medication related to prostate.

• Spinal card injury.

• Anxiety.

Treatment can be made easy if we know the cause of the issue. In general, medication will not be of much use in this situation. The procedures that can work are,

• Penile vibratory stimulation.

• Treatment for retrograde ejaculation.
