Hi doctor,

I am a male of 6 feet and one inch tall, weighing 205 lbs. I was diagnosed with caffeine-triggered anxiety six months ago. I had two episodes where I felt shaky, nauseous, and like I was on the verge of passing out. On both occasions, I had recently taken Tramadol and consumed an energy drink. Since those episodes, I constantly feel like I am in a dream. My memory is fine, but things just do not seem right. I randomly experience chest pain, and my vision becomes weird. My doctor tested for diabetes, and the results came back fine. I seem to feel worse after any caffeine, so I limit myself to one cup of coffee in the morning and a can of soda with my lunch. Do you agree that this being related to anxiety? The feeling does not seem to go away unless I am extremely focused on doing something. What else could be going on?

You had two episodes of vomiting. Both times you had taken a Tramadol, which is known to cause vomiting. However, the condition on both occasions was aggravated by rapidly consuming an energy drink. It can happen to a normal person, and Tramadol can cause vomiting even without energy drinks. Now, you have anxiety, and the caffeine present in coffee can increase heart rate. In someone with existing anxiety, this can make you feel worse. I would suggest you stop drinking coffee, and you may need SSRI (selective serotonin reuptake inhibitors) drugs to control your anxiety and address symptoms of depression. If your symptoms do not improve, consult a psychologist, and you should start feeling better.
