Hello doctor,
I have a lip infection with a bump. Lip cracks have already been there for over a year and I consulted a doctor. They suggested Chaptex lip balm but the result was not good. Now, a bump has appeared about one day back. Kindly help.

I have gone through your query and attachment (attachment removed to protect the patient's identity). This bump on the inner side of your lips is called aphthous ulcers (simple mouth ulcers). Many factors can cause ulcers in the mouth. It could be because of vitamin deficiencies, hormonal changes, psychological stress, allergies, accidental injury during brushing, food poking, or bacterial infection. This may take a week to resolve. This is absolutely normal. I suggest you do the following-
1. Avoid hot and spicy foods until the ulcer heals.
2. Do warm saline mouth rinses thrice daily.
3. Rinse your mouth after every meal.
4. If required, if it hurts, apply a topical anesthetic like Dologel (Benzalkonium chloride, Choline salicylate, and Lignocaine hydrochloride) or Hexigel (Chlorhexidine gluconate) over the ulcer with a clean finger.
Maintaining good oral hygiene and following the above-mentioned instructions will prevent further infection of the ulcer and promote faster healing.
Chapped lips occur when-
1. Our body becomes highly dehydrated.
2. Exposure to long-term cold weather or air-conditioned environment and direct sun.
3. Constant lip-licking or wetting habit (which makes our lips even drier).
4. Mouth breathing habit (if constant nose blocks occur).
This can be taken care of by following simple tips at home-
1. Drink lots of water and keep yourself hydrated at all times.
2. Use honey or sugar to exfoliate the dried skin over the lips. Applying lip protection creams over dried and flaky skin will have no effect. So rub those over the lips to remove dried flakes of skin and then apply lip protection creams for best results.
3. You can apply coconut oil over the lips for faster healing.
4. If present, it is best to break the lip licking and mouth breathing habit.

Which should I take- Dologel or Hexigel? And should I take any lip balm like Chaptex? Is it compulsory not to take milk or coffee? Kindly suggest.

Both Dologel (Benzalkonium chloride, Choline salicylate, and Lignocaine hydrochloride) or Hexigel (Chlorhexidine gluconate) provide the same results. Whichever is available in your place, you can buy. If you are planning to buy, I suggest you wear adequate protection before stepping out in this current pandemic scenario. Applying coconut oil will be sufficient. But if you are going out, it is better to apply Chaptex (Octyl Methoxycinnamate, Octocrylene, Lanolin alcohol, Avobenzone, almond oil), or other lip protection creams because it is easy to carry and apply. You can have milk and coffee. But it is better to limit the intake. If you wish to have, you can have coffee a maximum of twice per day.
