Hello doctor,

I have noticed two brown vertical lines on my left ring finger, particularly under my acrylic nail. It seems to be present only on this finger. Currently, I am taking birth control medication and Phentermine. Kindly help.

Hi,

Welcome to icliniq.com.

Based on your descriptions and the provided information (attachment removed to protect the patient's identity), it seems that you might be dealing with onychomycosis, a fungal infection affecting the toenails or fingernails, which can involve various parts of the nail unit, including the matrix, bed, or plate.

To screen for fungi, direct microscopy of a 20 % potassium hydroxide (KOH) preparation can be conducted. Medications for onychomycosis can be administered either topically or orally. A combination of topical and systemic treatments has been known to increase the cure rate.

The recommended treatment plan could involve the use of Efinaconazole 10 % topical solution or Amorolfine nail lacquer solution applied twice a week, along with Itraconazole 200 mg taken orally twice daily for seven consecutive days in a month for three consecutive months.

It is important to take all medications only after consulting with your general practitioner or a nearby dermatologist. Regular follow-up appointments should be scheduled, either after one month or as and when needed, to monitor the progress of the treatment.

I hope this helps. Feel free to reach out if you have any further questions.

Thank you.
