Hi doctor,

I have been having nasal bleeding for more than two years. The bleeding comes on and off. The bleeding is frequent on hotter days. I have profuse bleeding even while going to sleep. I have had medications for the same, but they do not seem to be effective. Please help.

Nasal bleeding or epistaxis at this age can have a variety of reasons. The most common reason could be nasal picking by the kid using his finger or some other object. Next, it could be due to intense crusting inside the nose due to some infective pathology. It can also be a sign of some granulomatous inflammation. I suggest taking the kid to an ENT (ear, nose, and throat) specialist, where there is a facility for nasal endoscopy and a laboratory. You will need an endoscopic examination of the nose by the otolaryngologist and also some blood investigations to rule out any bleeding diathesis.

In the event of an acute bleed, pinch the kid's nose gently for a few minutes. This will occlude the bleeding vessel and stop it. If it still does not stop, occlude the vessel that is bleeding and stop it. If it still does not stop, apply two drops of vasoconstrictor (decongestant) nasal drops and pinch. In any case, an evaluation with an otolaryngologist is necessary for further course.
