Hello doctor,

I have been experiencing severe lower back pain on the left side for the past two days. The pain was triggered after carrying a heavy bag. I am having difficulty finding a comfortable sleeping position and experiencing excruciating pain in the same area while walking. Kindly help.

Please follow the below instructions:

  1. Avoid lifting heavy weights.

  2. Avoid arching your back; instead, bend your knees to pick up objects from the floor.

  3. Avoid prolonged sitting.

  4. Avoid riding two-wheelers.

  5. Improve your sitting posture.

  6. Take short breaks every 20 minutes while sitting, and ensure your chair provides adequate back support.

  7. When getting up from a lying position, turn to the side before standing up.

  8. Consider bed rest if the pain is severe.

  9. Apply hot compresses locally over the affected area at least twice daily.

  10. Apply Oxalgin gel topically twice daily.

Medication regimen:

  1. Pregabalin (75 mg) tablet: 0-0-1 before food for 10 days.

  2. Ibuprofen (400 mg) tablet: 1-0-1 after food for 10 days.

  3. Rabeprazole (20 mg) tablet: 1-0-1 before food for 10 days.

  4. Shelcal (500 mg) tablet (calcium): 1-0-1 for 30 days before food.

  5. Vitamin D (60K units): Once daily for 10 days, then once a month for 11 months.

Please take these tablets only if you are not allergic to them and avoid them if you have bronchial asthma. Seek immediate medical attention if you experience any adverse reactions.

Physiotherapy:

  1. McKenzie's back extension exercises.

  2. Back core strengthening exercises.

  3. Glutei strengthening exercises.

  4. Passive straight leg raising exercises.

Start physiotherapy once the pain subsides. Regular exercises are crucial for effective results. After pain reduction, consider swimming, yoga, and walking, but avoid jogging, running, and weightlifting as they may exacerbate back problems.
