How is overactive bladder managed in teenage girls with ADHD?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My niece is 17 and has been dealing with urinary urgency and occasional bedwetting, which is really affecting her confidence. She also has ADHD and takes Lisdexamfetamine.

  • Could the medication be worsening the bladder issues?
  • What is the safest approach for a teenage girl with this combination?

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

Thank you for sharing your concerns.

I know how discouraging and frustrating urinary symptoms can be, especially for a teen trying to fit in at school, socially, and in day-to-day life. These are more common than most families realize, so please do not feel alone in this.

Lisdexamfetamine is a stimulant medication used to treat ADHD (attention-deficit/hyperactivity disorder). Usually doesn’t directly affect the bladder but can, in some teens, increase nervous system activity, anxiety, or affect sleep patterns.

Indirectly, this may contribute to a sense of urgency, frequent urination, or even bedwetting. Some teenagers also become less aware of bladder signals during the day while focused on activities, which can sometimes play a role.

There are also a few other things worth considering:

  • Constipation is very common in children and teens with ADHD and can put pressure on the bladder, sometimes causing symptoms that look like an overactive bladder.
  • Certain foods and drinks, such as caffeinated sodas, energy drinks, artificial sweeteners, or acidic foods, may also irritate the bladder.
  • Sleep quality and the timing of fluid intake during the evening can make nighttime symptoms worse as well.
  • If you haven’t already, a urine test may help rule out infection. Keeping a bladder diary for a few days (fluids, bathroom visits, leaks, and triggers) can also be helpful. If symptoms continue, a pelvic ultrasound and uroflowmetry may provide more information.

From a managerial perspective, simple behavioral measures are often very effective:

  • Timed bathroom visits every two to three hours, reducing fluids before bedtime, treating constipation if present, and limiting caffeine or fizzy drinks can all help.
  • Some families also find bedwetting alarms helpful.

If the symptoms don’t get better, her doctor may suggest medicines that can help. The good news is that these problems can be treated, and many teens get better with the right help and support.

I hope my answer is clear to you.

Revert in case of further queries.

Thank you.
