Hello doctor,

I am a 23-years-old very active athletic trainer. I am having anterior shoulder pain which is constant and severe sometimes. I have had an MRI, cortisone injections, taken NSAIDs, and physiotherapy with no relief in symptoms. I am wondering what the next step may be? I have read that a biceps tenodesis procedure would help relieve my symptoms, and I am curious as to know in what conditions this procedure is applied, because I know it is not just for a ruptured biceps. I also have decreased range of motion (ROM) with painful MMT (manual muscle testing). I had a labrum and RTC repair a few years ago. Kindly suggest what will be helpful.

I have read your query and I would like to know:

1. What is the exact site of pain?

2. Is it more on overhead movements?

3. Is that pain transient and sharp stabbing in nature or dull and aching?

4. Are there any night symptoms?

5. Is there pain in activities of daily living, if yes which ones?

6. What is the MRI (magnetic resonance imaging) report telling us?

7. Is there any thickening or fraying biceps?

8. What was the response to the steroid injection?

After reading your query without the above information, all I can understand is that the shoulder pain you have is because of impingement which is a common cause of any shoulder pain. And I will not jump to any conclusion to repair the biceps as I feel you are too young to have biceps rupture but tendonitis is a possibility and that should respond to a directed injection and some supervised physiotherapy. If you can help me with the above information I will be able to help you further.
