Hi doctor,

I am having a gastric problem for the past seven years. My stomach feels bloated and heavy after taking some food items. I have lactose intolerance for the past five years and milk products cause gastric problems immediately. I have this similar problem after taking oily foods also. It was occurring occasionally earlier but now it occurs more frequently. I am working from home and I sit for seven to eight hours daily in front of my computer. I usually sleep around 2 am in the night and I will wake up at 10 am in the morning.

I usually drink black tea with ginger after breakfast, coffee or tea without milk in the afternoon, and hot water with black pepper after dinner. I usually take my breakfast at 10:30 am lunch at 1:30 pm, and dinner at 10 pm. Sometimes I will take some snacks at 2 am. I am taking Eno and Pan-D when I feel bloated. After taking medicines, I have some relief. I am getting gastric episodes frequently for a year. I am taking Pan-D on empty stomach for ten to twelve days to cure this condition. I am having this problem for twenty days now.

I have bloating even after lunch or any time in the day now. I am taking Eno frequently. What is the exact cause of this gastric issue? What is the medicine to be taken? Why do I have it frequently? Do I need to do some tests? Is there any natural way to cure the problem? Can I have any side effects due to taking Eno and Pan-D frequently?

I understand your concern. You seem to have functional dyspepsia. As you are having problems for seven years, it is good to undergo an endoscopy soon. You have less chance to have irritatable bowel syndrome. There is no cure for it but it can be controlled. Some patients need to take medicines for lifelong to keep their symptoms under control. It is a diet and lifestyle disorder along with an altered gut-brain axis.

You need to do some baseline investigations such as CBC (complete blood count), LFT (liver function test), RFT (renal function test), FBS (fasting blood sugar), PPBS (post-prandial blood sugar), CRP (C-reactive protein), stool examination, endoscopy, colonoscopy and OGD (OesophagoGastroDuodenoscopy).

Usually, there is no natural remedy but some reports suggest that taking peppermint helps a lot. There are significant side effects due to these medicines but it is better to take them a few times a week. Try to avoid taking ginger and pepper frequently as they are gastric irritants.

I have done some blood tests and stool tests as suggested by you and I have attached them. Do I have any problem? Please suggest. Kindly suggest a medicine to get cured. Do I need to do an endoscopy? Is it fine to do an ultrasound?

Your blood reports and stool reports are fine (attachment removed to protect patient identity). At present, you can continue taking the same medicine. You can take Pan-D (Pantoprazole) for getting relief. You need to do an endoscopy to confirm the diagnosis of functional dyspepsia.

I am not willing to do an endoscopy as it involves a painful procedure. What causes gastritis in me even after having normal blood and stool report? Can I have a chance of developing life-threatening diseases like cancer? Is it good to do an endoscopy to confirm dyspepsia? Can I have other possiblities? Is there an alternative for an endoscopy? How long do I need to take Pan-D on an empty stomach? I consulted a doctor earlier and he suggested some medicines. The doctor suggested me to take Rifaxamine 550 mg for ten days and Levosulphride 25 mg for 15 days. Do these medicines help me?

Endoscopy is a five-minute procedure and it is absolutely risk-free if it is done by an experienced person. It can be done under sedation. You can have other possibilities such as H-pylori (helicobacter pylori), auto-immune gastritis, and eosinophilic gastritis. You have less chance to have cancer. But it can be ruled out by undergoing endoscopy. It helps in diagnosing functional dyspepsia. If it is normal, then medications can be taken for controlling your symptoms. There is no alternative for endoscopy. You can take Pan-D till you get relief from your symptoms.

Do I need to do tests for H-pylori? Does H-pylori antigen test is different from stool examination? Does routine stool examination is enough? Can I have any side effects due to Rifaxamine and Levosulphride tablet? Can I take Rifaxamine before a meal?
