Hi doctor,

I tripped into solar light in the backyard before Christmas, and a month later, I had a flu virus, and my calf muscle hurt when I put weight on it. I went to the emergency room and was admitted for five days. The doctor found a blood clot in the lung. I was placed on Xarelto and took a larger dose for 21 days. Then last two months, I was on the lower dose. Should I need to continue Xarelto? What was the reason for going to be admitted to the hospital? It lasted about seven weeks. The main symptom is weakness. Please help.

From your description, it seems like deep vein thrombosis and pulmonary embolism. This is the first episode. I agree with the treatment plan, and it can be continued for three months if there is no underlying predisposing factor. D-dimer is expected to be high in this condition.

Then there is no need to taper off the Xarelto after I finish the last pills. Should I still take the D-dimer test soon to see if the clot is gone or have an image of the lung to be certain? Will the vein or vessel be normal again? Please help.

You can stop after three months of anticoagulation, no need to repeat the D-dimer test. The veins will be recanalized even after a few weeks of anticoagulation. Only you have to take prophylactic anticoagulation in the future in case of any predisposing condition like prolonged immobility etc.
