Hello doctor,

My father is 74 years old. He met with a road accident about 3.5 years back and had suffered a brain injury. A craniotomy and cranioplasty were performed. Since then, he has been in a persistent vegetative state. He is completely paralyzed and continuously on nursing care. He is fed by a PEG tube. He is also using a suprapubic catheter and the breathing is through tracheostomy. We keep changing the suprapubic catheter at home once in 15 days. Today, while changing the suprapubic catheter in the morning, during the catheter insertion, there was lot of blood that came out from the normal urinary route and also from the catheter opening. We immediately removed the catheter and put a condom catheter. We increased his water intake now. Since morning, he has passed urine approximately 15 times. When he starts to urinate, he gets blood in the beginning and then it is clear and towards the end he gets almost 0.16 ounce of blood. At the time of insertion, approximately 0.5 to 06 ounce of blood oozed out. Now, should we wait for the fever to start and then give IV antibiotics or shall we start IV antibiotics now? What antibiotics can we give? When can we send urine for culture any other specific information?

Relax and do not worry. The reason for bleeding might be due to a small injury while changing the suprapubic catheter. You can go to some local doctor or can call a doctor at your home and ask him to change the suprapubic catheter. I think antibiotics are not required at present. Since he was on a long-term catheter, there are high chances that there will be some infection in urine, but it will be a subclinical infection. You will be unnecessarily treating it with antibiotics, which are not required. If he gets a fever, then do a urine culture. Otherwise, urine culture is not required.
