Hi doctor,

I am a 31-year-old and currently breastfeeding my 11-month-old baby. Blood in my stool started after a hard stool, and then I checked with the doctor, and he told me it was a fissure. Then I used cremaffin, and it went away and again started last month. A few days back, I went to the doctor, and he told me it was a fissure. But for the last few days, I have had lower abdomen pain like menstrual cramps (but no periods), and today when I wiped after stool, I saw a small clot on the tissue. Can this be cancer?

I understand your concern.

I have reviewed your attached images (attachments removed to protect the patient's identity). A few more details need to be sent. Is the blood mixed with stools, or is it fresh blood streaks in stools? The description is likely secondary to a fissure in the anal region, aggravated by constipation or hard stools. Get yourself well-hydrated. Avoid constipating food. Take Cremaffin (Milk of magnesia) whenever you notice stools becoming harder or you need to strain. You should apply Diltiazem (Diltigesic) or Fidonal (Lidocaine) cream in the perianal region to promote healing and reduce pain. You can consider a colonoscopy if this recurs and once the fissure heals. Consult your specialist doctor, talk with them, and take medications with their consent.

Thanks for the reply.

The blood is either streaked on stool or tissue when I wipe doctor. I am worried as I have lower abdominal pain now. It is in the pelvic region. Can a fissure cause small blood clots, as I showed in the picture?

Yes, fissures can cause similar clots. Pain needs evaluation. A colonoscopy scan is further recommended.

I got my first period after my pregnancy doctor. not sure if that is the cause of my abdominal pain for past 4 days(As I always have had menstrual cramps from beginning) Doctor also yesterday night I took milk of magnesia and today I had a loose stools till i had so much pain while passing the stool and also blood, I am just so worried seeing blood and also for the pain , i am so scared to go to a BM

Take laxatives so as to soften stools. Pain may be due to menstrual cramps
