Hello doctor,

I have had bad body odor for five years despite normal hygiene. A test from the doctors came back with no infection markers and no liver issues. My diet is plain, and I have tried everything to get rid of it, such as Benzoyl peroxide wash, Zinc and Magnesium supplements, high-strength antiperspirants, different diets, and soaps, etc.

Kindly help.

I am really sorry for what you are going through, and I can completely understand your issue. This is a very common outpatient department (OPD) complaint that we often encounter.

Persistent body odor (bromhidrosis) despite good hygiene can be frustrating and often has multiple possible causes. Since you mention that the odor has been present for about five years, occurs even immediately after showering, and there is no visible rash or infection, the issue may be related to apocrine sweat gland activity rather than infection.

The armpits contain apocrine glands whose secretions can be broken down by skin bacteria, producing a strong odor even when the skin appears normal. Sometimes routine washing or shaving does not fully control this because the bacteria reside deeper in hair follicles and sweat glands.

The approach is to reduce both bacterial load and sweating. Washing the underarm area once daily with a Benzoyl peroxide cleanser to decrease odor-producing bacteria is helpful, but it should be left on the skin for a few minutes before rinsing. In addition, use a strong antiperspirant containing Aluminum chloride hexahydrate at night to reduce sweat production, which will decrease odor formation.

Certain medical and metabolic conditions may also contribute to unusual body odor. A rare condition called trimethylaminuria can cause a persistent odor that does not respond well to normal hygiene measures.

Hormonal changes, diet (especially foods rich in sulfur compounds such as garlic or onions), stress, and some medications may also influence body odor. Although your liver and infection tests were normal, if the odor is very strong or unusual, discussing further metabolic testing can be helpful.

Persistent body odor could be due to sweat gland activity, bacterial imbalance, diet, medication effects, or a metabolic cause.
