Hello doctor,

I have been in a lot of discomfort when I go to the bathroom for the last week. The pain is intense, and I experience a cutting feeling at the anal hole as I pass stool. Then I get a scorching feeling that can last for a while.

I have been having a hard time going to the bathroom. It is a lot of time spent in the bathroom with discomfort. Finally had a bowel movement today, although it was a challenging process.

I have seen no bleeding and no obvious bulge. The constant ache has been harming my health in general, and I have been feeling weak and worried about what it might be.

I read your query. I understand your concern about anal fissure.

Your symptoms are similar to what happens when you have an anal fissure. This is a tear in the lining of your anus. It often causes pain during a bowel movement followed by a burning feeling that may continue. The common causes are constipation and passing hard stools.

Not seeing blood does not mean you don't have a fissure. The main goal now is to keep your stools soft and prevent straining during bowel movements. You might want to drink water eat more fruits, vegetables and other foods high in fiber and use a stool softener like polyethylene glycol.

Taking baths for 10 to 15 minutes three to four times a day especially after bowel movements may help reduce pain and promote healing. Try not to sit on the toilet for periods and don't try to force the stool out.

After a check-up your doctor may suggest topical treatments to help heal and relieve discomfort. These include an anesthetic cream or prescription drugs such, as Diltiazem or Nitroglycerin ointment.

If you don't feel better in two to three weeks if your symptoms come back often or if you have bleeding, swelling, fever, increasing constipation or extreme discomfort see your doctor for an examination. This will help confirm the diagnosis and rule out any disorders affecting your anus or rectal area.
