Does straining during bowel movement cause vaginal pressure?

Hello doctor,

I am a 29-year-old female. I had a difficult bowel movement a few days ago, and ever since, I feel a heaviness in my vagina that is very uncomfortable. I am too scared to look down there, and I do not know what to do to make it go away.

I am deeply concerned about your worries. Based on your symptoms, the discomfort and heaviness in your vagina after a difficult bowel movement could most possibly be due to, and it is most consistent with, pelvic floor strain or mild pelvic organ prolapse.

Your symptoms may appear due to recent straining during bowel movement, resulting in pressure on pelvic muscles, and the feeling of heaviness or pressure in the vagina. It is common in mild prolapse or muscle fatigue. They are usually not painful, and no visible abnormality is initially visible.

However, a few other possible causes, like vaginal or pelvic mass (rare if no other symptoms like bleeding or discharge) and pelvic muscle spasm or tension after straining, should also be considered.

However, what you can do now is:

  • Avoid straining: Make bowel movements easier by adding more fiber-rich food, drinking plenty of water, and doing gentle activity.

  • Do pelvic floor exercises (Kegels): This will strengthen support muscles.

  • Also monitor symptoms and look for pain, bleeding, bulging tissue, or worsening heaviness.

  • Seek medical evaluation if you have a feeling of tissue bulging out, persistent heaviness or pain, or urinary or bowel changes.

However, mild pelvic floor strain after constipation is common and usually temporary. Most cases improve within days to weeks with rest and exercise.

Do not worry, your symptoms could most likely be due to mild pelvic floor strain or early prolapse caused by straining. Conservative care and monitoring are appropriate, but see a gynecologist if symptoms worsen or new signs appear.
