Hello doctor,

I had protected sex with a sex worker 56 days ago, but I have been experiencing a burning sensation in my penis for the past month. I have attached the test reports. What other tests and treatments should I undergo?

I have reviewed the history and images (attachments removed to protect the patient's identity) provided from your end. The report shows IgG (immunoglobulin G) positive HSV (herpes simplex virus) 1. IgG HSV 1 positive depicts the presence of herpes infection in the past, and IgM (immunoglobulin M) negative represents the absence of current active herpes infection. Since you do not have any symptoms, medications are usually not needed. However, in case of symptoms, it is advised to take medication. I would also like to mention that herpes does not have a cure and may appear on and off. It may remain dormant in your body, showing symptoms following infection. In addition, the herpes virus may sometimes remain dormant within the system.

My suggestions:

  1. Tablet Acyclovir 200 mg is to be taken every four hours, five times a day, for ten days after breakfast.
  2. Tablet Ascazin (Zinc acetate) 50 mg is to be taken twice daily after breakfast and dinner to prevent further recurrences to a certain extent by providing immunity.
  3. Lifestyle modifications can be done by increasing protein, fruits, and vegetables and consuming at least 14 glasses of water daily to strengthen immunity. In addition, avoiding oily and fried food, milk and dairy products, red meat, and sweets would help maintain immunity and prevent recurrences.

Apart from the above, the test reports also show positive trichomonas in urine, which are most commonly seen among women and may be sexually transmitted to males as well. They present with a burning sensation during urination, pain, and discharge in the urethral area, which may be foul-smelling. Since you have mentioned these symptoms, kindly take the tablet Metronidazole 2 g (five tablets of 400 mg to be taken together with water) after breakfast. It is to be taken as a single dose which means five tablets in total.

The test report also shows Chlamydia pneumoniae which is a respiratory infection. Therefore, I request you consult a pulmonologist for treatment and management of the same. As I have mentioned, due to the possibility of recurrence of STI (sexually transmitted infection), it is always advised to follow a diet to prevent recurrences. Do not worry. The above medications would help with the symptoms and reduce the load of urinary bacteria. Consult a specialist doctor, discuss with them, and take medications with their consent.
