Hi doctor,

I have chestburn after eating food. I also have breathlessness even after taking a full breath. Sometimes it feels like a pinch on the left and right sides of the chest. What should I do?

Based on what you have shared, it appears you are dealing with gastroesophageal reflux disease (GERD), which occurs when acid from the stomach flows back into the esophagus after eating. Additionally, the presence of a hiatal hernia, where the upper part of the stomach bulges through the diaphragm, is common in individuals who are overweight and can worsen symptoms like heartburn and difficulty breathing.

To confirm these issues and rule out other conditions, your doctor may recommend investigations such as an electrocardiogram (ECG), chest X-ray to assess your lungs and diaphragm, upper gastrointestinal endoscopy or barium swallow to examine the esophagus and stomach, and blood tests including complete blood count (CBC), kidney and liver function tests, and thyroid function tests.

Lifestyle changes are essential in managing your symptoms. You should avoid heavy meals and refrain from lying down immediately after eating. Raising the head of your bed by 4 to 6 inches can also help prevent acid from rising. Weight loss, even as little as 5 to 10 kilograms, can significantly improve symptoms. Your diet should exclude spicy, fatty, and acidic foods, as well as caffeine and carbonated drinks. Instead, opt for smaller, more frequent meals.

For medical management, antacids such as aluminum hydroxide with Magnesium hydroxide (Gaviscon or Gelusil) can provide quick relief. In addition, a proton pump inhibitor like Pantoprazole should be taken twice a day, once before breakfast and once before dinner, for a duration of two to four weeks, half an hour before meals.
