Hello doctor,
My 72-year-old father, an Alzheimer's patient, has pain and itching on his left leg and one of the fingers of the leg. In addition, the skin is dry and hollow on the second finger of the leg. I have attached the pictures of his leg and finger for your reference. Can you please suggest the diagnosis of his skin problem and treatment for the same. Thank you.

I have seen the attachments (attachments removed to protect the patient's identity) and understand your concern. The condition looks like a callus. I suggest you help your father to follow the below treatment plan. Soak his toe in water for 20 minutes and remove the extra thick skin using a callus remover. Apply Soframycin skin cream (Framycetin Sulphate 1 % w/w) on Sunday and Monday, every morning and night. Apply Dersol 30 % ointment (Salicylic Acid) from Tuesday to Saturday on the affected area of the skin once at night. I hope this helps. Kindly revert in case of further questions. Thank you.

Thank you for your answer. Last week I consulted you for my father's skin complaint. As per your suggestion, we are applying cream Moisturex (Lactic acid, Light liquid paraffin, Propylene glycol, and Urea). The callus is improving; however, black spots have started to spread to the entire leg. I have attached some pictures for your reference. Please suggest.

Use Cream Moisturex (Lactic acid, Light liquid paraffin, Propylene glycol, and Urea) to be applied on the entire leg, in the morning and at night. Ointment Propysalic NF 6 (Clobetasol Propionate and Salicylic Acid) to be applied on the lesion every morning and night. Take Tablet Bilashine (Bilastine) 20 mg SOS for itching. I hope this helps.
