Hello doctor,

I have had a 5 mm hypoechoic left nodule for two years. FNAC was done recently in Bethesda G3. After FNAC, a second nodule of 3 mm appeared. No microcalcification or vascularity. Cervical nodes TIRAD3. I Was on thyroxine 25 mcg. Recently stopped medicine, TSH is getting high from 5.5 to 8. Repeat USG? Echogenic focus in Right paratracheal node. I have been suggested both follow-up and surgery. Please suggest.

I just read your query and the ultrasound and FNAC reports (attachments removed for patient identity protection).

The FNAC report suggests that the possibility of papillary carcinoma cannot be ruled out, which means they are not sure it is papillary carcinoma. Still, it looks like one, according to them. In such a diagnosis, it is usually better to have a thyroidectomy done rather than waiting for the likely carcinoma to spread. So, I suggest you either have an FNAC done in another extremely good laboratory or go ahead as per your doctor's advice for surgery.
