Hello doctor,

I had a chemical injury to my eye, which got resolved with steroid treatment. The doctor said to put ointment in every night. I got new glasses last week, and he said that it was like taking paint off a door. I have taken a film off my eye. He said I would need to put drops for a lifetime. My understanding was that my eye had resolved and would heal in time. Please give your opinion.

What happens in chemical injury is that the acid or alkali penetrates deep into the eye's tissue, reaching to the lens and angle of the eye at times as well. Since your vision has resolved, there is least likely that a deep penetration has occurred. Nevertheless, there is a covering over the eye that is the tear film and numerous glands on the surface that produce the tear film layers. These often get disrupted during a chemical injury beyond repair. And for these, we need to put artificial tears or lubricating drops for a long period. Sometimes for scarred tissue, steroids are given for over six months to lessen the scar.

I have noticed that I do not have the same clarity of vision when I am reading. Is this an issue with dry eyes? I put the Hylo Forte regularly into my eyes. I am worried about my vision now, which has never happened to me before. I also notice a slight difference or irritation, and I need to close my eyes for a second to alleviate it. Please guide me.

This is definitely due to dry eyes. I suggest you kindly start the following medications. Take a tablet of Doxycycline 100 mg twice a day, and a tablet of vitamin C 400 mg three times a day. Apply eye ointment Genteal gel (Carboxymethylcell-hypromellose) three times a day. Continue with Hylo Forte (sodium hyaluronate). Do warm compresses over the eyelid for five minutes twice a day. This will take time to heal. Dry eyes take months to settle.

Yes, it takes time for the tear film and glands to replenish post chemical injury. Once the glands and tear film get back to normal, you will be relieved. You take these medications for 15 days and let me know.

Yes, Doxycycline will help collagen remodulation in the cornea and surrounding tissue, which gets damaged in chemical injury. Hylo drops are plain Sodium Hyaluronate which compensates only the lipid layer. Genteal gel is Carboxymethylcell-hypromellose. It not only works on lipid but also aqueous layer and helps in replenishment of corneal surface. It has great results, so I do recommend it. Take Hylo drops six times a day. Not just at night. If you have improvement, then it is a good sign. Dry eyes take around three to six months to recover. Please mention whether you have completed taking a course of Doxycycline. Dry eyes will be there for your entire lifetime. You need to put a lubricant occasionally for a gritty feeling.

Glaucoma can be prevented when detected on time. There are good medicines available. Please mention whether you have used Naphazoline drops or Phenylepherine for redness. These medicines are effective.

It causes blanching of vessels so redness goes away and gives a cooling effect as well. I suggest you take Doxycycline 100 mg twice a day for a month and tablet vitamin C 400 mg four times a day. Continue using lubricants. Add Zoline plus drops (Camphor, Menthol, Hydroxypropylmethylcellulose, and Naphazoline) four times a day.

I had a chemical injury to my eye as per previous communication. I have been using the daily drops and warm compress twice and still have dry eyes. I had my eyes checked by a dry eye clinic and they recommended that I do intense pulse light treatment. I do not know if I am doing more harm than good. I have one more treatment left. The ophthalmologist said that I would be conscious of my eyes which I am, it is giving me anxiety. Is this the glands that are causing me this sensation? Am I going to have this discomfort all the time?

Please do not panic. Intense pulse light therapy is an advanced therapy for very severe dry eye which has shown improvement in cases not responding to medical treatment. Please continue as your doctor has suggested.

You should be using sodium hyaluronate 0.18 % rather than simple 0.1% which is there in Hylo drops. Also add eye ointment Genteal gel at bed time. That will be helpful.

Is it called Sodium Hyaluronate eye drops? Why have I such discomfort in my eyes? Will they ever return to normal or is they the way they are going to be? Please do not panic. See the chemical injury does some amount of permanent damage. The medications help. Replenish things to 60 to 70 %. Yes you can increase omega 3 fatty acids. You can take safely up to 3 months.
